Deerfield Daycare Registration Requirements

In addition to the paperwork below, to complete your child's enrollment at our Deerfield early learning center, your child will need the following documentation:

  • Copy of Birth Certificate
  • Most Recent Medical (on DHS School Physical Form)
  • Copy of Current Vaccination Record
