Ready to move forward with our school family? We encourage interested families to give us a call to ensure that we are a good fit for your family, we can meet each other face-to-face, we can answer all your questions, and to make sure that we have space/availability for your needs.

In addition to the paperwork below, your child will need the following documentation:

• Copy of birth certificate
• Most recent medical (on DHS School Physical Form)
• Copy of current vaccination record
